Consent Form
The Consent form grants the Institute permission to conduct the screening and to share the information collected with the coaching staff, and appropriate medical personnel attending the athlete. In addition, it allows use of the physical data in research performed by the Institute and it's parent organization, The Hughston Foundation. The form MUST be completed and signed by a parent or guardian prior to conducting the screening.
