Physical Form
The physical form is a three-page form to collect medical information on the student athlete.

The first page is to be filled out by the athlete before arriving at the Pre-Participation Screening site. It is often helpful for the athlete's parent(s) to assist in filling out the form, as questions about illnesses and injuries address the lifelong history of the athlete. The second and third page will be filled out by the Pre-Participation Screening personnel as the athlete moves to each station. However, the athlete needs to fill out appropriate information prior to the Screening (i.e. name, date of birth, age, sex, and emergency information).

DO NOT fill out any information regarding: 1. The physical evaluation itself or 2. The clearance for participation. These may only be completed by the Pre-Participation Screening personnel.
